NEW AND RETURNING ATHLETE INFORMATION

All paperwork (including 2 copies of the front and back of your health insurance card) must be completed and returned to the Sports Medicine Office by July 18, 2012.  Please click on the forms below, complete them, print them and return them to:

*The Student-Athlete Credential will be given to your professors, along with your athletic event/missed class schedule, to aid you in introducing yourself to the instructor and pave the way to a successful semester.

All student-athletes must complete a yearly pre-participation physical exam (PPE) through the KSU Sports Medicine Dept.  A PPE with your personal physician will not be accepted.  The PPE schedule*** is as follows:

Tues., July 31, 2012         Soccer/Cross Country
Tues., August 7, 2012      Volleyball/Cross Country
Tues., August 14, 2012    Non-Fall Sports – To Be Determined
Wed., August 15, 2012     Non-Fall Sports – To Be Determined

**** Appointments will be scheduled for each athlete.  Athletes will be notified of their appointments at a later date.  If unavailable for the assigned date, athletes must notify Sports Medicine immediately (770-423-6487 or ycombs@kennesaw.edu).  A charge of $25 will be collected by the KSU Health Clinic, if the athletic physical is not completed on one of the 3 dates listed above.  Most of the “Non-Fall Sports” physicals will be conducted on Tues., 8/14, so make appropriate travel plans.
